We accept appointment requests by telephone, fax, or e-mail from test participants and/or their legal guardians, legal representatives or referring agency. However, to best ensure that we are provided with the complete and correct identification and contact information, we recommend that the adults who will be tested or who will be providing consent for testing of a minor child or legally-incompetent adult contact us directly. Appointments are usually available the same day or with a few days advance notice.
